announced that Forest Hill Family Health Associates has expanded its existing partnership
with the company to include care coordination services and solutions.
Forest Hill, a comprehensive primary care center based in Newark, N.J., is an NCQA
recognized Patient Centered Medical Home, which is already part of Horizon Healthcare
Innovations, Inc. Patient Centered Medical Home payment model reform initiative.
The practice was recently selected by Horizon NJ Health, a Medicaid managed care
organization, as the first practice to pilot its Medicaid Patient Centered Medical
Home Initiative.
This has presented a unique challenge for the practice to manage healthcare for
a wide array of plan beneficiaries while catering to a multitude of quality indicators
for different payers. As a result, Forest Hill required a uniform way of providing
care coordination and quality reporting.

the individual physicians, specific populations, clinical and utilization outcomes
and quality of care.
As a Horizon BCBS and Horizon NJ Health featured PCMH program site, Forest Hill
has a sponsored embedded population care coordinator who is tracking and reporting
on HEDIS and other required quality and utilization metrics. To date, the PatientPoint®
platform has significantly increased the productivity and patient reach of this
population care coordinator.
