ORLANDO, Fla. - September 11, 2012 — PatientPoint®, the leader and innovator in care coordination solutions that drive engagement with patients and providers, today announced that Cornerstone Health Care, the Triad's largest independent physicians group serving more than 80 locations in North Carolina, will be deploying the PatientPoint patient engagement platform to enhance the patient experience, while also aligning front office and back office workflows to provide more cohesive care throughout the continuum.
During the first phase of a multi-phase implementation, PatientPoint will deploy electronic point-of-care check-in/check-out devices to streamline administrative processes, perform medication management, administer screenings and assessments for identified patients, and render patient satisfaction surveys. PatientPoint is also helping Cornerstone develop clinical workflows for its care team members within the patient-centered medical home (PCMH) and accountable care organization (ACO) models by delivering content directly through its patient monitoring dashboard and to the group's electronic medical records (EMR). As a result, physicians can review patient data quickly and easily at the point of care, and within their already-established workflow, rather than having to leave their workflow to find and review the data manually.

PatientPoint technology supports Cornerstone as it moves toward the ACO model under the Medicare Shared Savings Program. Through this program, Cornerstone will work with CMS to provide Medicare fee-for-service beneficiaries with high quality service and care, while reducing the growth in Medicare expenditures through enhanced care coordination.
As a true leader in the physician group space, Cornerstone also has future plans to partner with other physician groups to assist in making the transition from the traditional fee-for-service model to newer fee-for-value based care models.
