NCQA Prevalidates the PatientPoint® Care Coordination Platform for PCMH 2011

CINCINNATI, Ohio - February 13, 2013 — PatientPoint®, the leader and innovator in care coordination solutions that drive engagement with patients and providers, today announced that the PatientPoint Care Coordination Platform is the first mobile-enabled care coordination and patient engagement platform to be prevalidated by the National Committee for Quality Assurance (NCQA) for 2011 patient-centered medical home (PCMH) criteria .

NCQA conducted a formal review of the PatientPoint Care Coordination Platform as it relates to PCMH 2011 requirements and awarded PCMH prevalidation with approved autocredit for functionality that completely meets specific factors within PCMH 2011 standards and guidelines. The PatientPoint Care Coordination Platform has been prevalidated for 16 factors with an emphasis on patient access and continuity, population management, care coordination and medication management. As a result, approved autocredit can be transferred to practices utilizing the PatientPoint prevalidated functionality to help achieve NCQA PCMH recognition.

Established patient-centered medical homes have demonstrated improved quality outcomes and reductions in the total cost of care. According to the Patient Centered Primary Care Collaborative's (PCPCC) 2012 Review of Cost & Quality Results, several PCMH organizations reported lowering the cost of care by up to 20 percent and reducing hospitalizations and ER visitations by up to 50 percent. By having software prevalidated by NCQA, PatientPoint is poised to drive continued growth and transformation of physician practices to PCMH and other accountable care initiatives.

The PatientPoint Care Coordination Platform automates many of the processes required to more meaningfully coordinate and manage care of the patient population. By aggregating data from EMRs, health plans and patient self-reported information, the platform is able to identify and reconcile gaps in patient care. Leveraging innovative patient engagement tactics including IVR, secured messaging, mobile applications and tablet based check-in, the PatientPoint platform automates the pre-care outreach to noncompliant patients on behalf of the physician, streamlines clinical processes at the point of care, and facilitates patient adherence to required medications and compliance to care plans post care. As a result, physician practices seeking to operationalize PCMH and ACO can gain up to 30 percent more in clinical and administrative efficiencies.
