revenue cycle management solutions, today announced that Medinova Physicians LLC has expanded its existing
partnership with the company to include care coordination technology and National Committee for
Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) consulting services.
The Bronx, NY-based, five location practice specializes in family and internal medicine.
Medinova selected PatientPoint® to assist in its mission of achieving Level 3 PCMH recognition,
which is the highest NCQA designation.
The practice will deploy PatientPoint's HealthSync Care Coordination Platform, which will integrate with
its existing practice management and electronic health records (EHR) systems. The platform automates
care coordination processes and simplifies patient engagement through the following:
By leveraging this approach, Medinova will be able to better satisfy PCMH requirements and move from its
current Level 1 PCMH recognition to Level 3. For example, Medinova will be able to identify and address
care gaps for patients with chronic conditions such as hyperlipidemia, hypertension and diabetes through
an automated process. The practice will then monitor and report on this population health information and
related improvements—a key component to NCQA certification.
