Orlando, Fla., February 8, 2012 — PatientPoint®, a provider of
care coordination and patient engagement management solutions, today announced a
strategic partnership with ACME MSO
to provide automated care coordination services for its Clinical Value Network (CVN) of providers.
Since its founding in 2001, ACME MSO has provided turnkey management services to IPAs that
serve thousands of greater Chicago area residents. These IPAs have come together to form CVN,
a network of more than 100 physicians to manage and administer care for Blue Cross and Blue
Shield of Illinois and Wellcare Harmony members. This new operating model requires providers
to be more accountable - both clinically and financially - for patient care. As a result,
it is imperative to proactively identify and streamline the coordination of preventative
care measures.
PatientPoint's HealthSync Care Coordination Platform will serve as the technology
backbone for this important initiative—helping CVN-affiliated physicians align,
analyze and positively influence patient behavior across the network. Also, by
leveraging the HealthSync Platform, CVN providers will be able to certify for
NCQA recognition and pursue emerging accountable care opportunities such as the
Patient Centered Medical Home.

PatientPoint technology will be integrated with existing managed care systems to
produce alerts for medication management and preventative screenings, enabling member
IPAs to meet core clinical measures required for their patient populations. The platform
will aggregate and reconcile data from electronic health records (EHRs) in combination
with patient self-reported data to produce relevant gaps in care. Patients are then alerted
of the gaps on behalf of their physicians via phone or IVR, secure text or email.
As patients arrive for their scheduled appointments, comprehensive clinical summaries
are rendered for both the provider and patient—allowing for a more informed consultation
and intervention when appropriate.
As CVN physicians provide care, information is transmitted from the provider's interaction - along
with any additional patient self-reported results - back to ACME MSO case managers so they can
support proper care plan adherence and ensure patient compliance. This automated care coordination
process allows the physician, IPA, and health plan to:
- Jointly address gaps in care;
- Drive better patient adherence;
- Enable plan and provider cost savings;
- Minimize adverse events and ED visits; and
- Streamline the coordination of care.
